Community Employment

The Workshop's Vocational Programs Meet These Types of Rehabilitation Needs:

❏ Undetermined employment goals

Community Integrated Employment Programs and Services:

All programs are designed to assist individuals to obtain and retain community integrated employment. Assistance provided is determined by the level of support required of the applicant and can include but not limited to:

❏ Determining what types of jobs interest the applicant
❏ Resume development
❏ Application completion
❏ Interviewing skills / practice interviews
❏ Pre-job testing assistance
❏ On the job training

The length of job coaching services is dependent on the program referred to and the level of support needed to get and maintain community integrated employment.

1915i Supported Employment:

Provided through Medicaid Home and Community Based Services (HCBS) (must have active NC Medicaid). Service can occur individually or in a group, 1915i Supported Employment is a Community Based Service. Job coaching staff can stay with the person served as long as support is required to maintain the person’s employment and personal safety and well-being.

Admission Procedure for 1915i:
❏ Applicants must be referred by an LME/MCO or other funding source that is contracted with The Workshop of Davidson, Inc. 1915i has dual referral requirements
❏ Applicants must contact an LME/MCO (such as Partner’s) to request a care coordinator be assigned to the applicant for completion of the prerequisite 1915i assessment
❏ Applicants will then request services from EIPD to include a referral to The Workshop of Davidson

Innovations Supported Employment:

Community Based Service provided through a Medicaid funded waiver, (must have active NC Medicaid and be assigned a waiver slot by an LME/MCO to be eligible.) Service can occur individually or in a group. Job coaching staff can stay with the person served as long as support is required to maintain the person’s employment and personal safety and well-being.

Admission Criteria & Eligibility Requirements:
❏ Age requirements vary by program and are 18 years of age minimum
❏ Intellectual / Developmental Disability or Traumatic Brain Injury
❏ Applicants who exhibit aggressive or dangerous behaviors may not be accepted
❏ If applicants experience seizures, they must be under control
❏ Applicants must be able to physically and emotionally care for their needs (to include but not limited to: ambulation, eating, dressing, bathing and toileting) Unless 1 on 1 waiver services are available
❏ Applicants must be willing to participate in assigned program

Required information includes:
❏ Application
❏ Current physical exam
❏ Current psychological report with adaptive testing and definitive IDD or TBI diagnosis
❏ Health/Social/Vocational history
❏ Current sources of identification to include but not limited to: Valid NCID card, Social Security Card, Birth Certificate, etc.
❏ All applicants will be screened by the admissions committee. Those found ineligible may be referred to other agencies for services.

Division of Employment and Independence for People with Disabilities (EIPD)

EIPD Supported Employment: Community Based Service for persons requiring support that includes long term follow-up by a job coach. Applicants will apply for services through EIPD who will then make referrals to The Workshop of Davidson. After successful placement and training, regular job coaching ends after a 90-day phase out period and then transitions to twice monthly long-term follow up job coaching.
EIPD Work Adjustment: Community Based Service for applicants that do not need extensive or long-term job coaching support. After successful placement and training Work Adjustment job coaching ends after a 90 day phase out period.

Long Term Vocational Supports/Long Term Follow Up: Community Based Service for EIPD referrals who do not need extensive ongoing support can be provided Long Term Follow after regular job coaching ceases. This service provides a check in with the employee and employer twice monthly to trouble shoot issues or provide other supports to assist with job continuance.
